Especially for our specialized treatments, we require records from a previous provider, preferably using this fillable PDF referral form. Insurance may fully cover Spravato™ and TMS if at least 2 antidepressants have failed (dx F33.2), but not ketamine.

Patients with active mania or seizures and those with neurovascular compromise won’t qualify for TMS, ketamine, or Spravato.™

Along with the patient’s last medical note, referrals can be emailed to newpatients (@ascendhealthcenter) or faxed to (833) 974-2062. The goal is to utilize the best local resources and avoid the “dead end” patients describe of having to start over each time they try a different approach. We work well with outside providers to leverage our expertise and can provide additional interventions (such as vitamin infusions or SPG blocks) for their patients at their request. We strive to build strong relationships with local medical and therapy practices so that patients receive the collaborative care they deserve.
